Before taking the Air Force Physical Fitness Test, you must complete an Air Force Fitness Screening Questionnaire. The form is required prior to taking the test and is intended to help ensure your health and safety. We’ve all heard stories about someone suffering a heart attack on the 1.5 mile run, so this form is the Air Force’s way of identifying those who are at high risk due to factors like age, smoking, diabetes, high blood pressure, etc. that are at an increased risk of injury or death during the test.
In addition, you will need to fill out an Air Force Physical Fitness Test Scorecard, which captures your performance on each exercise (waist measurement, push ups, sit ups, and run) and documents your final score.
If you are on a physical profile exempting you from one of the testing components, then you need to have an AF Form 422 in order to test.
